You can only use this form to request prior tax year transcripts if you file your taxes based on the calendar year. If you file taxes based on a fiscal year that is not from January 1 to December 31st, you cannot use 4506T-EZ which is the short form. For non calendar year filings you must use Form 4506T (the long form).
Instructions for Completing Form 4506T-EZ
Completing the form is relatively simple. In boxes 1a and 1b insert the name as shown on the tax return and Social Security number or tax ID. In box 2a and 2b insert the name of your spouse (for joint tax returns) and of course the spouse’s Social Security number or taxpayer identification number.
Next in boxes 3 and 4, complete the demographic information such as the address, city, state and zip code. Include apartment number if applicable. If the address used for the prior tax return is different from your current address, insert the prior mailing address information in box 4 on Form 4560T-EZ.
Third Party Authorization
If you wish to send the tax returns to a third party, for instance a mortgage company or an accountant, insert the name, address and phone number of the third party in the boxes provided for question number 5.
Tell the IRS which transcript years you need. In the boxes provided in question number 6, list as many as four prior tax years. All you need to enter is the 4-digit year. No need to list Jan, 2008 thru Dec, 2008. The transcripts will be processed within 10 business days.
Sign and Mail Form 4506T-EZ
Lastly, sign and date Form 4506T-EZ and mail it to the appropriate Internal Revenue Service office.
Make sure to review the instructions on page 2 of Form 4506T-EZ. There is where you’ll find the proper mailing address for your state. These tax forms are processed regionally so you must make sure to send your request to the proper regional office.
